Private-pay care is built around consistent, meaningful visits, with a minimum of four hours per care day and eight total hours per week.

What happens after I submit a referral?
Susan, Ehren, or the SILK team reviews the information and, with permission, contacts the family. Together, we explore the right support, timing, and payment path. A start date is confirmed after the care plan and dependable schedule are in place.
How early should I refer?
Refer as soon as the daily-living gap becomes clear, even if a discharge date or long-term plan is still being finalized. Early contact gives the family more time to understand options.
What information should I send?
Send professional and family contact details, county, expected timing, a general description of the daily-living help being considered, and confirmation that the family agreed to be contacted.
Can SILK update the referring professional?
With the family’s permission and an appropriate communication method, SILK can let the referring professional know whether contact occurred and next-step planning began.

SILK provides authorized Medicaid/PASSPORT services in Gallia, Jackson, Pike, Vinton, and Ross Counties. SILK welcomes private-pay inquiries across Southern Ohio, with a four-hour minimum per care day and eight total hours per week.
